The Blue Cross Short-Term Medical PPO plan for individuals and families
is now available! This plan meets your immediate needs for healthcare
coverage, and may be the right solution if you find yourself without medical
insurance because you are between jobs, waiting for permanent coverage,
or have recently graduated from college.
Coverage is for periods of 30 to 185 days with a choice of deductibles from
$250 to $2,000. You may elect any day of the month to begin or end coverage.
Pricing is based on a per member per day rate, and the premium must be
pre-paid with a check or credit card (Visa, Mastercard, or Discover).
You can benefit from Blue Cross Life & Health's lower negotiated medical
costs and one of the largest PPO networks in the state. You'll also have
access to Blue Cross Life & Health's wealth of value-added programs.
What the Plan Covers
Access to any doctor (but you'll pay more for services of
non-participating providers)
Emergency care
Professional services including x-ray, lab, and office visits
Prescription Drugs
Hospitalization services
Outpatient services
Ambulance ($1,000 is maximum coverage)
Accidental death and dismemberment coverage up to $50,000
Healthy Extensions, which includes preventive care information and
information about discounts offered by independent vendors on
alternative health and wellness products and services
Preventive health screenings at HealthyCheck Centers
24-hour medical information and resources through MedCall
Discounts on vision care, exams, LASIK, lenses, and frames
through HealthyExtensions
$3,000,000 per person policy maximum benefit for medical
BlueCard access to participating doctors and medical facilities
throughout the United States at Blue Cross negotiated rates
