No annual medical or brand-name prescription drug deductible
An office visit option for Preventive Care benefits
$25 copayment and/or 30% coinsurance for most in-network services
Graduated Out-of-Pocket Maximum
The Advantage Plan takes an entirely new and innovative approach to the
structure of the out-of-pocket maximum. Instead of paying a flat percentage
up to a given maximum, the Advantage Plan pays on three levels for in-network
services:
70% up to $900 in member out-of-pocket fees
90% between $901 and $3,600 in member out-of-pocket fees
100% after $3,600
Why It's Called the Advantage Plan
The graduated out-of-pocket maximum approach can cause less
financial stress to members by breaking up their share of
costs over a longer period of time.
Blue Cross Life & Health will pay a higher share of costs
sooner than other plans.
Members pay 30% of costs until they hit $900, and then only
10% until they hit the out-of-pocket maximum of $3,600.
